DR. DOMINIC CHAN - BSC, DC

Dr. Chan has been in clinical practice in the Commercial Drive area of East Vancouver for over a decade. In that time, he has had the pleasure of meeting and treating numerous patients from all walks of life.
After growing up in the Lower Mainland and then earning his Science degree from UBC, Dr. Chan pursued his doctorate at the University of Western States in Portland, Oregon. Combining his strong science background with his love for sports, Chiropractic was a perfect fit. Dr. Chan has seen patients in all conditions benefit from treatment catered to athletes. His education and experience has proven that an active approach to care is not only backed by science, but the most efficient way for people to regain their health.
His years of clinical experience and eagerness to teach has made him a well referred professional in the field of Chiropractic. Humbled by many satisfied patients, he has been voted the #1 Chiropractor in the City of Vancouver. With a vision to maintain a high level of evidence based care for all patients, Dr. Chan has sought to bring talented professionals together under the Peakform Wellness banner.
In his spare time, Dr. Chan remains active and continues to participate in many sports including; running, triathlons, volleyball and swimming, to name a few.

DR. ALEXANDER LEE - BSC, DC
CHIROPRACTOR
Dr. Alexander Lee is a Vancouverite native and grew up in Richmond, BC. He holds a Bachelor of Science degree in Kinesiology with a concentration in Active Health and Rehabilitation from Simon Fraser University. He then pursued his Doctorate in Chiropractic from Palmer College of Chiropractic in San Jose, California.
Dr. Lee uses his expertise and experience in diverse chiropractic manipulative techniques, exercise, and movement rehabilitation. He is certified in Full Body IASTM (Instrument Assisted Soft Tissue Mobilization) and Functional Movement Systems to provide quality personalized care in order to relieve pain, restore function, and promote wellness. He understands that no two bodies are the same and that every patient needs personalized treatment plans that cater to their specific injuries and diagnoses. Whether you are an athlete, business professional, student or weekend warrior, Dr. Lee takes a hands-on approach to your health to help clients get back to doing the things they love – pain free.
During his spare time, Dr. Lee is a wrist watch and automotive enthusiast and enjoys all things mechanical. He likes to keep active by going to the gym regularly and playing recreational dodgeball.

DR. NICK CRIVICI - DC
CHIROPRACTOR
Dr. Nick Crivici graduated summa cum laude from the University of Western States in Portland, Oregon with a Doctor of Chiropractic degree and a Bachelor of Science in Human Biology. Prior to this, he attended Simon Fraser University and graduated with a Bachelor of Science in Kinesiology, Concentration in Active Health and Rehabilitation and a Minor in Psychology.
Dr. Nick utilizes an evidence-based approach to care as he reads the latest articles and stays up to date on current chiropractic literature. He employs a patient-centered, whole-body approach to care and takes the time to understand your needs and goals for treatment.
In his spare time, you will find Dr. Nick riding his motorcycle on a winding road or the racetrack, harvesting some fresh vegetables from the garden or working on a DIY project at home.

DR. DONNY MACKINTOSH - BKin, DC
CHIROPRACTOR
Dr. Mackintosh grew up in the Lower mainland and graduated with a Bachelors of Kinesiology degree from UBC. He then went on to pursue his Doctor of Chiropractic from Canadian Memorial Chiropractic College in Toronto. Dr. Mackintosh believes in an evidence based approach to patient care that includes; joint manipulation, soft tissue therapies, stretching, rehabilitation exercises and lifestyle modifications, just to name a few. When not treating patients, Dr. Mackintosh enjoys fishing and staying active by running and working out. He’s also a big Canucks and Blue Jays fans. Having grown up playing organized baseball at high levels, athletic performance is of huge interest to Donny.

MICHELLE CHAN - RMT

Michelle graduated from the 3000 hour program at Vancouver College of Massage Therapy (VCMT) in Dec 2015, where she is also a clinic instructor and teachers assistant. She was drawn to the massage profession because of her love for people and passion for helping them. After experiencing how massage can aid in the healing of chronic injury and stress, she felt inspired to help others in similar situations.
Michelle is dedicated to assisting clients with their musculoskeletal health in a non-judgemental, open and safe environment. She strives to identify the underlying cause of pain and dysfunction while treating the mind, body and spirit as a whole. She achieves this by using a mix of therapeutic and relaxation techniques such as:
• Myofascial release, Deep tissue massage, Trigger point release, Active and passive stretch, Neuromuscular therapy, Scar tissue treatment, Swedish massage, Soft Tissue Release
In her spare time Michelle is an avid snowboarder and enjoys being out in nature hiking, camping and relaxing. She also enjoys traveling, good food and having a balanced lifestyle.

Allison uses a combination of Swedish massage, IASTM (instrument assisted soft tissue mobilization), as well as more active techniques and myofascial to treat common issues such as back and neck pain and more serious conditions including impingement, whiplash, disc herniations and more. Having experienced chronic pain herself, Allison understands the importance of making small changes in daily habits to help decrease stress or pain conditions. She also looks forward to creating tailored care plans to help her patients combat these issues and achieve their health goals.
On her free time, Allison enjoys dance and swimming as she believes freedom of motion and mobility are very important for an active healthy lifestyle, as well as for everyday life.

I-TING HUANG - R.Ac.
REGISTERED ACUPUNCTURIST
I-Ting Huang graduated from Vancouver Career College where he earned his diploma in acupuncture. He is now a registered acupuncturist (R.Ac) with the College of Traditional Chinese Medicine Practitioners and Acupuncturists of British Columbia (CTCMA), I-Ting was born and grew up in Taipei, Taiwan and moved to Canada in 2020. He also holds a Bachelor of Science in Plant Pathology and Microbiology from National Taiwan University.
In his treatment, he incorporates acupressure tuina technique and fire cupping with acupuncture to treat pain, sport injuries, stress, insomnia, depression, skin issues and many more.
