FREQUENTLY ASKED QUESTIONS

HERE ARE ANSWERS TO COMMONLY ASKED QUESTIONS AT PEAKFORM, INCLUDING OUR RATES. 

DO YOU DO DIRECT BILLING?

Yes! Please scroll down to see a list of some of the companies we directly bill to. We will need your Policy and Identification number to submit your claim. Please note we will only be able to directly bill to the primary policy holder on any one visit. If you have any questions, please feel free to call our office. 

In order to be eligible for Direct Billing, patients must provide their Member ID and Policy under any of the above-mentioned plans. Unfortunately, we cannot search patients without their member and policy information. The direct billing portal doesn't allow for it nor do they provide for any extensive plan information. It is imperative that patients have this information with them when they come to the clinic for initial set-up.

Insurance plans only allow direct billing to the primary plan.(We do not bill the 'balance' of treatment cost to a secondary spousal plan. This is an insurer policy and not a clinic policy. 

Please also note, any cancellation fees incurred by patients must be paid out of pocket and can not be billed to insurance.

DO YOU TAKE ICBC
OR WORKSAFE PATIENTS?

Yes! Both of our clinics have clinicians who can treat ICBC or Worksafe patients.  Please have your current, valid claim number ready and we can directly bill ICBC for 25  Chiropractic visits, 12 Registered Massage Therapy sessions and 12 Acupuncture sessions. (Please note for extended Registered Massage Therapy sessions, a user fee may apply)

With a current, valid claim number, we can also accept and bill Worksafe for treatments as well. 

WHAT IS YOUR CANCELLATION POLICY?

We require a minimum of 24 hours notice for cancellation of all appointments/sessions.

 

Also, in order to keep patients and staff safe by limiting the time spent at the front reception area, we also require a Credit Card to be kept securely on file. Please note, there are only two ways we can accept cancellations or re-scheduling requests.

1) The patient can use the online booking app to make the cancellation themselves, as long as there is at least 24 hours left until the apprtake cancellation requests left on voicemail or email. You can also cancel your appointment through Jane App with a minimum of 24 hours notice.

Cancellations require a payment of a cancellation fee equal to the value of service at full cost.

Our cancellation policy is in place to ensure that time slots are given to those in need of our services and so that we can help as many patients as we can.

WHAT CAN I EXPECT FOR MY FIRST TREATMENT?

We will first assess your medical history, and proceed to work out a treatment plan for your unique situation. It is a mix of both a health consultation and a treatment. You're more than welcome to bring a friend or family into the room with you!

WHAT ARE YOUR RATES FOR TREATMENTS?

CHIROPRACTIC CARE:

                                          Initial Treatment:           $100

                                          Subsequent Visit:            $65

                                          Extended Treatment:    $100

For ICBC and Worksafe patients, please call us so we can verify coverage over the phone before your first appointment. After that, we can offer treatment with no 'out of pocket' expenses)

REGISTERED MASSAGE THERAPY:

                                          Initial Treatment (60 Min) :     $125

                                          Subsequent Visit (60 Min):     $125

                                          Subsequent Visit (45 Min):     $100

                                          Subsequent Visit (75 Min):     $145

                                          Subsequent Visit (90 Min):     $175

ACUPUNCTURE:
